Fishbone

An Ishikawa diagram for one stubborn problem: causes sorted along bones like people, process, tools, environment. Structured root-cause hunting for recurring issues.

Steps

  1. 10 min
    Define the problem

Agree the problem statement; write it at the fish's head.

  2. 10 min
    Draw the bones

Pick 4–6 cause categories: people, process, tools, environment…

  3. 25 min
    Brainstorm causes

Fill each bone with candidate causes; ask 'why' to go deeper.

  4. 20 min
    Deep dive

Vote the most likely causes; test them against evidence from the sprint.

  5. 10 min
    Decide actions

Agree countermeasures for the top verified causes, with owners.
